For those (the majority, I assume) of you who don't know what that is, it's a pen and paper RPG based on the classic ADOM Rogue-like computer game, which was a crowdfunded Kickstarter add-on / stretch goal of a campaign for a new version of ADOM with improved graphics and UI and stuff.
Yeah, and the computer version has many other possibilities and they're often game-changers . . . sometimes when I've gotten corrupted, it has been quite useful, at least for a while . . . though they pretty much all reduce your APpearance, which affects your social interactions.Dang it! I don't have the time for a new rogue-like or another rpg I'd have to run to ever get to play... But those mutations look useful... Grrrr...

Home of the Brave for CP2020? And my latest challenges. Never realized how hard it was to find layouts that didn't have the game title in the header or footer of each page...

Okay, I feel obligated to give a couple of clues for my entry (post #219) since it's one of the main ones that has not yet been guessed. Which feels kinda odd, given how long I was out of gaming, that something from my meager collection is somehow stumping the lot of you (seriously, it's like seeing some kid on the playground running one past Michael Jordan).
It was released in two different formats: the first was just a standalone booklet, the second is was turned into a box set, with some extras added, a year later.

And in other news, I think I've finally identified Tulpa Girl's obscure entry. Based on a review in the Space Gamer, it must be The Castle Perilous by James Sheldon, published by West Wind Simulations. I've never seen that game, but it fits the publication history (initial printing followed by box set, company only published this game) and the review notes that game includes fiction and "bracket numbers" for combat, as in the example given above.
This one had me stumped for a good while, since it seems like Chivalry & Sorcery (and even references it) but is different from the first and second editions, which are all I'm familiar with.And another offering (the image has some blurriness from compression, sorry):

Looking at reviews, though, it's probably for the 3rd edition, since that used the PSF (Personal Success Factor) mentioned in this screenshot. I'd guess the Creatures Bestiary, given the page numbers.
One more and then I'll stop, for a while at least. Sorry about the items used to hold the book open; there is also a little redaction.

It occurs to me that one of the harder things to identify may be retro-clones, since there are so many of them and their rules are so similar. Here are a couple of examples: (first one guessed and removed)
